Wording for Affidavits

I recently had a client with two affidavits in relation to her request for permission, as a bankrupt, to be self-employed. The second affidavit was an annexation to the first one and in support of the application. However the wording on top of the second affidavit did not comply with the required wording, not having her address and occupation or the words ‘swear that the information and statements contained in this application are true and correct’ or similar.
She claimed that the form was provided by the NZ Insolvency and Trustee Service but I doubt it.
I refused to take the affidavit.
